Applying takes on a few minutes, but before you begin, note the requirements below:

  • I am a practice owner OR I am an employee of a Provider Organization.

  • I have an NPI number.

  • I have a current application on CAQH (Council for Affordable Quality Healthcare) or OHP (OneHealthPort) for Providers in the state of Washington

Alternate Method

Alternatively, if would like to create or maintain a CAQH profile please visit ada.org/godigital if you are a dental provider or, if you are a medical provider please visit this link. Once a providers CAQH profile is complete, the level of maintenance is very low. Providers will only need to re-attest once every 120 days and update application information only when a change occurs. An online single source application offers great time saving and prevents repeated application needs for health plans.

During the credentialing and re-credentialing process, HeadsUp Healthcare and its partners will obtain information from various sources to evaluate applications. Providers and practitioners have the right to review any primary source information that is collected during this process, however, this does not include the release of references, recommendations or other information that is peer review protected.
